Community Development Block Grant (CDBG) Recipient Information

Requesting Grant Reimbursement

Recipients of CDBG grant funds must submit invoices and proof of payment prior to receiving reimbursement. Invoices should include the business name and address and a clear description of the items purchased. Proof of payment should be in the form of a cancelled check or credit card statement. Cash payments are discouraged and may not be reimbursed.

You are only allowed to request up to three (3) reimbursements. Draws will be submitted to the state once every three weeks. Receipts are due one week prior to the submission date. Please see the schedule below. A printable version of this schedule can be downloaded here.

All receipts should be submitted with a cover page that lists the invoices being turned in for payment. Grant recipients may use their own spreadsheet, or download this template for use.

When possible, please submit invoices and proof of payment digitally. If hard copies must be submitted please make photocopies and retain your originals. No staples please.

Purchase of electronics including televisions, computers, iPads, etc. should be discussed with Livingston County Economic Development prior to order. If approved, all electronics invoices must be accompanied by a letter from the business owner describing the items purchased and how they will be used, and confirming that the items are for business purposes only and will remain on premise.

Documenting Job Creation

Employers should complete this Job Creation Worksheet for each created position or title. A full job description, as advertised, should be attached to the Job Creation Worksheet along with any documentation of how a job was advertised, including copies of any online listings or posts.

Each individual that is hired should complete a Family Income Form. The forms are based on the location of the individual’s residence, as opposed to the business location. If you need a Family Income Form for a county that is not listed here, please contact our office.
